In choosing to participate in Awakening Dharma offerings, I accept full responsibility for myself during those offerings and waive all liability from Stephen Snyder and Awakening Dharma and all officers, directors, employees, or agents of Awakening Dharma for all personal result or outcomes. I understand Stephen Snyder and Awakening Dharma are not offering professional, medical, or psychological advice and this program is not a substitute for consultation with a healthcare professional. I accept all risks inherent in such a dharma offering and agree to hold, and indemnify, Stephen Snyder, Awakening Dharma, and its officers and directors, employees, or agents harmless.
Audio/Video Recording Release
In choosing to participate in Awakening Dharma offerings, I, hereby release my rights of privacy and agree that any audio or video recording can be used by Awakening Dharma or Stephen Mugen Snyder, Roshi for teaching purposes.
